Supporting Older Australians to Live Well at Home
From 1 November 2025, the Australian Government introduced the Support at Home program — a simpler, fairer, and more flexible way for older Australians to receive care and support in their own homes.

The Support at Home program replaces the previous Home Care Packages (HCP) and Commonwealth Home Support Programme (CHSP), bringing them together under one streamlined system. This means less complexity, more choice, and greater control over how your care and funding are managed.

Designed for Independence, Dignity and Choice

The new Support at Home model is built around you — your needs, preferences, and rights. It focuses on providing the right support at the right time, so you can stay active, confident, and engaged in your community.

Services are flexible and tailored, allowing you to access:

  • Clinical care and nursing support
  • Everyday living assistance
  • Social and community participation
  • Assistive technology and home modifications
  • End of life and palliative care

There are six categories of services you can receive via Support at Home:

  1. Clinical supports: Services delivered or supervised by qualified health professionals to maintain or regain functional or cognitive capabilities.
  2. Independence: Support for personal care, social engagement, respite, transport and other activities that help older people manage daily living and remain independent. •
  3. Everyday living: Domestic assistance, home maintenance, meal preparation and other services that help participants maintain their home environment and everyday routines.
  4. Assistive technology and home modification (AT-HM) scheme: A dedicated pathway providing equipment, products and home modifications to improve safety, accessibility and independence for older people living at home.
  5. End-of-Life Pathway: A short-term pathway providing enhanced in-home support services for older people with a prognosis of three months or less who wish to remain at home.
  6. Restorative Care Pathway: A time-limited, goal-oriented pathway to help older people regain or maintain their functional independence, thereby reducing their need for longer-term supports.

The Support at Home program has been developed alongside the new Aged Care Act, which takes a rights-based approach to care. This means your choices, dignity, and independence are protected by law.
