Understanding the Transitional Aged Care Program

The Transitional Aged Care Program (TACP) is specifically designed to help older people recover at home after being in hospital. TACPS provides essential case management, therapy support and services aimed at helping each person regain as much independence as possible. Services Included in TACP

Each person’s Transitional Aged Care Program package will be unique, delivering services and case management that tailored for them. Services that can be provided include nursing support, allied health supports (physiotherapy, occupational therapy, speech pathology, dietetics, social work and allied health assistants), help with cleaning, shopping, transport, meal preparation, and personal care. TACP packages can also provide respite and short term equipment loans.

Duration and Scope of TACP

The program is time-limited and typically runs for up to 12 weeks, starting from the day the person leaves the hospital. The duration is flexible and based on the individual’s specific needs. The primary focus during this period is to optimise the functional capacity and independence of the older person. This approach ensures that each individual has adequate time and support to complete their restorative process and adjust to their home environment post-hospitalisation.

Carexcell’s Implementation of the Transitional Aged Care Program

Carexcell collaborates with the Mid-North Coast Local Health District to provide the Transitional Aged Care Program. We work with the LHD to ensure that the person leaving hospital receives services as soon as possible following discharge. Carexcell also works with the Transitional Aged Care Program’s Case Managers to ensure that services continue to meet each person’s needs.
