As part of Australia’s aged care reforms, the Home Care Package (HCP) program is being discontinued. From 1 November 2025, individuals receiving a Home Care Package will be transitioned to the new Support at Home program.
What This Means for You
Support at Home is a government-funded aged care program that helps older Australians live independently in their own homes. It replaces the previous Home Care Package system. To learn more visit our Support at Home Page.

If you currently receive a Home Care Package, your services will continue as usual until the new system begins. You won’t lose support — instead, your plan will transition to the Support at Home model, ensuring your care remains continuous.
Your Carexcell team will work with you to review your supports and make sure your new plan reflects your preferences, goals, and daily needs.
You absolutely can. If you already receive services through Carexcell, we’ll automatically transition your care to the new Support at Home model.
There’s no need to reapply — and your familiar team of registered nurses and support staff will continue to provide your care.
